About the position

The Outreach Specialist will engage, assess, and support unhoused individuals in various living situations, including encampments and interim housing. Utilizing trauma-informed practices, the role focuses on providing intensive case management and crisis intervention, particularly for those living in RVs, campers, and trailers. The position requires collaboration with multiple organizations to ensure effective service connections and follow-up support for individuals experiencing homelessness.

Responsibilities

  • Provide mobile, field-based outreach to unhoused individuals in various settings.
  • Connect participants to mental health, health, housing, and other supportive services.
  • Engage in outreach services and community referrals to promote well-being.
  • Collaborate with other outreach entities in the community.
  • Provide crisis prevention, intervention, and de-escalation services as needed.
  • Deliver ongoing street-based housing-focused case management services.
  • Ensure referrals are completed and provide advocacy as needed.
  • Maintain thorough and accurate records in written form and electronic databases.
  • Attend regular staff, agency, and community meetings as designated by the supervisor.
  • Drive personal vehicle in and around Los Angeles County.

Requirements

  • Demonstrated knowledge of issues facing program participants such as health, substance abuse, and trauma.
  • Ability to demonstrate empathy and a non-judgmental attitude.
  • Possess a high level of tolerance for individuals with mental and physical health needs.
  • Flexibility to work in a changing environment.
  • Availability for after-hours crisis calls and disaster response.
  • Knowledge of community social service agencies and resources.
  • Conflict resolution skills and effective time management.
  • Strong written and verbal communication skills.
  • Experience in team environments and collaboration.
  • Maintain confidentiality according to HIPAA standards.
  • Obtain and maintain CPR/First Aid Certification.

Nice-to-haves

  • Bilingual: Spanish/English preferred.
  • One year of volunteer or work experience with unhoused individuals or in health counseling.
