About the position

The Outreach Specialist plays a vital role in the Supportive Services for Veteran Families program, focusing on assisting veterans and their families who are homeless or facing housing instability. This position involves outreach, engagement, and advocacy to connect veterans with necessary resources and support, ensuring their well-being and stability. The Outreach Specialist will work collaboratively with community partners and case managers to facilitate access to services and maintain supportive relationships with clients.

Responsibilities

  • Perform and lead outreach, engagement, and advocacy in all service areas for Supportive Services for Veteran Families.
  • Complete schedules, logs, and reports on outreach activities, including data on referral, intake, assessment, and disposition/assignment.
  • Complete all pre-screen, threshold, and enrollment forms and secure related eligibility documents for veteran clients.
  • Work collaboratively with VOMASS Case Managers to identify relevant service categories, options, and priorities.
  • Develop and maintain linkages and contacts with community partners and collaterals.
  • Build a strong network of referral sources to generate a quantifiable flow of referrals.
  • Participate regularly in team meetings and external meetings as assigned.
  • Perform related duties as required.

Requirements

  • Bachelor's degree or Associate's degree preferred; minimum of 2 years related experience in a case management role may be substituted.
  • Knowledge of veteran issues, homelessness, and state & Federal resources highly preferred.
