About the position
The Overnight Care Manager for the Mobile Crisis Response Team at Gracepoint is responsible for providing direct care to individuals with behavioral health needs. This role involves coordinating and ensuring continuity of evidence-based care through triage, assessment, planning, and communication. The Care Manager will serve as the primary dispatch during the overnight shift, working from 12 midnight to 8:30 am, Sunday through Thursday.
Responsibilities
- Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements.
- Complete all clinical and administrative documentation based on established timelines as per program requirements.
- Communicate effectively and maintain cooperative relationships with internal and external resources.
- Develop service plans, treatment plans and/or aftercare plans with clear goals and objectives as per program requirements.
- Maintain efficient workflow management and organization as per program requirements.
- Maintain proficient quality of clinical documentation as per program requirements.
- Other duties as assigned or needed as per program requirements.
Requirements
- Must have a Bachelor's Degree in Human Services or a related field of study toward Human Behavior/Science.
- Must be able to lift and carry up to 50lbs of dead weight.
- Must have excellent interpersonal skills.
- Valid Florida Driver's license, reliable personal transportation, and insurance.
