About the position

Corewell Health Surgical Transplant is seeking an Advanced Practice Provider (APP) to join a dedicated team that provides comprehensive surgical care to heart and lung transplant patients, as well as those requiring mechanical circulatory support. This role involves managing patient care across preoperative, postoperative, and outpatient settings, ensuring effective communication with multidisciplinary teams, and participating in various clinical meetings and rounds. The APP will not be involved in surgical procedures but will play a crucial role in patient assessments, care continuity, and family communication.

Responsibilities

  • Participate in daily rounds with the multidisciplinary team.
  • Conduct daily assessments of preoperative and postoperative transplant and VAD patients.
  • Maintain communication between the multidisciplinary team and have thorough knowledge of VAD and transplant patients.
  • Provide continuity of care for VAD and transplant patients throughout inpatient and outpatient settings.
  • Communicate with VAD and transplant patient families, including participation in family care conferences.
  • Represent the cardiothoracic service at VAD/heart transplant and lung transplant selection committee meetings.
  • Provide primary coverage for the cardiothoracic surgery service during the evaluation and workup of transplant and MCS patients.
  • Perform consults for VAD, heart transplant, and lung transplant evaluations.
  • Manage surgical and wound concerns in VAD and transplant patients in the Heart and Lung Transplant Clinic.
  • Prepare patients for transplant regarding the pre-transplant plan and checklist.
  • Maintain knowledge of patients on the heart and transplant waitlist and participate in weekly pre-heart transplant risk review meetings.
  • Participate in weekly VAD/Transplant MDT rounds and discharge planning meetings.
  • Manage chronic driveline infections, including monthly review meetings and frequent clinic appointments.
  • Perform procedures such as chest tube management and removal, wound care, and surgical site evaluation.

Requirements

  • Master's Degree in Nursing or Physician Assistant program from an accredited institution.
  • 3 years of relevant clinical experience.
  • Current Nurse Practitioner or Physician Assistant license in the State of Michigan.
  • Basic Life Support (BLS) certification.
  • Pediatric Advanced Life Support (PALS) certification.
  • Neonatal Resuscitation Program (NRP) certification.

Nice-to-haves

  • Familiarity with surgical anatomy and transplant procedures.
  • Working knowledge of physiologic consequences of heart and lung transplantation.
  • Experience with MCS technologies including ECMO, Impella, Heartmate II, III, Heartware, and Centrimag.
