About the position

The Lead Interdisciplinary Team (IDT) Moderator at San Ysidro Health is responsible for training, coaching, and identifying operational gaps within the IDT. This role focuses on enhancing patient satisfaction through effective training and EPIC optimization while ensuring compliance with regulatory requirements. The Lead IDT Moderator serves as a liaison between Center Management and IDT Moderators, facilitating communication and operational improvements.

Responsibilities

  • Collaborate with leadership to develop workflows, training programs, and EPIC optimizations that align with PACE regulations and policies.
  • Train and coach other IDT Moderators on workflows and role expectations.
  • Support the Center Manager in creating training materials as needed.
  • Ensure all team members are effectively fulfilling their assigned IDT roles.
  • Regularly report any conduct or behavioral issues among Moderators to the Center Manager.
  • Communicate operational issues and concerns to the Center Manager, focusing on continual program improvement.
  • Meet regularly with Site Dyad Leadership and Technician Managers to review goals, trends, and performance measures for each team member.
  • Promote the mission and vision of leadership positively and productively.
  • Coordinate the hiring, training, and evaluation of IDT members with the Technical Manager.
  • Provide input in annual performance reviews related to IDT and team performance.
  • Ensure team members understand the importance of their roles and remain open to teamwork.
  • Coordinate orientation and in-service programs for staff to meet regulatory requirements.
  • Ensure the provision of quality service within expected standards and financial targets.
  • Facilitate activities related to new participants, including orientation.
  • Coordinate all IDT Team Meetings, including care planning and implementation.
  • Participate in the continued development of the team to improve decision-making processes.
  • Monitor and track grievances, complaints, and dissatisfaction from participants to identify trends.

Requirements

  • Bachelor's degree in a healthcare-related field or equivalent work experience.
  • Minimum 2 years of experience working with an Interdisciplinary Team.
  • Minimum 2 years of experience working with the frail elderly population.
  • CA Driver's license with appropriate insurance coverage.
  • Good written and oral communication skills.
  • Technical knowledge necessary to provide care for geriatric patients.

Nice-to-haves

  • Experience with EPIC systems.
  • Knowledge of healthcare compliance and medical records.
