About the position

The IDT Moderator at San Ysidro Health plays a crucial role in ensuring that the Interdisciplinary Team (IDT) operates effectively, making decisions about medical, social, and supportive services based on the IDT decision-making process. This position requires a strong understanding of PACE regulatory knowledge and compliance with the San Diego PACE model, policies, and processes. The IDT Moderator will work closely with leadership and team members to promote collaboration, facilitate care planning, and ensure quality service delivery to participants.

Responsibilities

  • Meet regularly with Site Dyad Leadership and Technician Managers to review goals, trends, and performance measures for each team member.
  • Promote the mission and vision of leadership in a positive and productive manner.
  • Coordinate the hiring, training, and evaluation of IDT members with the Technician Manager.
  • Provide input in annual performance reviews related to IDT and team performance.
  • Ensure team members understand their roles and the importance of teamwork.
  • Coordinate orientation and in-service programs for staff to meet regulatory requirements.
  • Ensure quality services are provided within expected standards and financial targets.
  • Facilitate activities related to new participants, including orientation and care planning.
  • Coordinate all IDT Team Meetings, including care planning and implementation.
  • Monitor and track grievances and complaints from participants to identify trends.

Requirements

  • Bachelor's degree in a healthcare-related field or equivalent work experience.
  • Minimum 1 year of experience working with an Interdisciplinary Team.
  • Minimum of 2 years of experience working with the frail elderly population.
  • CA Driver's license with appropriate insurance coverage.
  • Good written and oral communication skills.
  • Knowledge of medical terminology and working knowledge of Microsoft Windows and MS-Office.
