About the position

The Personal Care Assistant II at Central Valley PACE Center in Modesto plays a crucial role in supporting the nursing staff by delivering patient care activities and tasks. This position is designed for individuals who are committed to the mission, vision, and values of Golden Valley Health Centers, focusing on providing high-quality care to participants in a day center environment. The role involves mentoring other PCAs, communicating patient needs, and assisting with various personal care tasks, all while working collaboratively within an interdisciplinary team.

Responsibilities

  • Acts as the mentor and Lead PCA.
  • Communicates patient care needs to the LVN and supervising RN.
  • Assists in training PCA's and other staff.
  • May assist in the therapies department.
  • May assist in the clinic escorting patients to and from appointments, taking vitals and prepping exam rooms as needed.
  • Provides basic personal care assistance and assistance with any other activities of daily living across settings to assigned participants.
  • Communicates effectively with participants, families, staff, other departments and agencies.
  • Works effectively in a team setting.
  • Documents treatments/procedures, observations, nursing care and patient activities on appropriate forms.
  • Participates in teaching/learning process with others.
  • Follows guest relations policies and procedures.
  • Abides by all established policies, rules and regulations.
  • Participates as a member of the interdisciplinary team (IDT).
  • Attends staff meetings and takes part in participant care planning.
  • Observes each participant for any change in physical, mental, emotional and social functioning and reports such changes to the licensed nurse.
  • Performs patient care activities appropriately, reporting any circumstances or change in patient's condition.
  • Other projects and duties as assigned.

Requirements

  • Valid California Driver's License, acceptable driving record and vehicle insurance.
  • Must be able to treat people of all ages including geriatrics.
  • Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently.
  • High School Diploma or equivalent required.
  • Current unencumbered CA licensure as a Certified Nurse Assistant, Home Health Aide, Emergency Medical Technician, Advanced Emergency Medical Technician or Paramedic; or certification from an accredited program for clinical medical assisting.
  • Current BLS CPR Card certified by the American Heart Association.
  • Prior personal care assistance experience, or ability to pass internal competencies within six (6) months of hire.
