About the position

The Personal Care Assistant II at Central Valley PACE Center in Modesto plays a crucial role in supporting the nursing staff by delivering patient care activities and assisting with daily living tasks. This full-time position operates Monday to Friday, focusing on providing quality care to participants in a home care setting. The role includes mentoring other PCA staff and participating in interdisciplinary team meetings to ensure comprehensive patient care.

Responsibilities

• Act as the mentor and Lead Patient Care Assistant (PCA).
• Communicate patient care needs to the LVN and supervising RN.
• Assist in training PCA's and other staff.
• May assist in the therapies department.
• Escort patients to and from appointments, take vitals, and prep exam rooms as needed.
• Provide basic personal care assistance and assistance with activities of daily living.
• Document treatments/procedures, observations, nursing care, and patient activities on appropriate forms.
• Participate in the teaching/learning process with others.
• Follow guest relations policies and procedures.
• Observe each participant for any changes in physical, mental, emotional, and social functioning and report to the licensed nurse.
• Work effectively in a team setting and communicate effectively with participants, families, staff, and other departments.

Requirements

• Valid California Driver's License with an acceptable driving record and vehicle insurance.
• High School Diploma or equivalent required.
• Current unencumbered CA licensure as a Certified Nurse Assistant, Home Health Aide, Emergency Medical Technician, Advanced Emergency Medical Technician, or Paramedic.
• Certification from an accredited program for clinical medical assisting.
• Current BLS CPR Card certified by the American Heart Association.
• Prior personal care assistance experience or ability to pass internal competencies within six months of hire.
