About the position

The PACE Social Worker at NeighborHealth plays a crucial role in supporting adults aged 55 and older within the Neighborhood PACE program. This position involves working as part of a dynamic Interdisciplinary Team (IDT) to develop and implement care plans that help participants live safely in their communities. The role includes conducting psychosocial assessments, providing counseling, and coordinating care to address various issues such as mental health, medical decision-making, and caregiver support.

Responsibilities

  • Conduct psychosocial assessments and identify factors impacting participants' functioning.
  • Complete initial assessments and contribute to care plans to enhance participants' well-being.
  • Provide caregiver support and lead family meetings.
  • Actively participate in the Interdisciplinary Team, sharing assessments and collaborating on care plans.
  • Develop and lead activities that support participants' goals.

Requirements

  • Master's degree in Social Work (MSW) is required.
  • Passion for serving adults aged 55 and older.
  • Experience working with older adults is preferred.
  • Must possess or be working toward licensure as a Licensed Certified Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) in Massachusetts.

Nice-to-haves

  • Bilingual Spanish language skills are preferred but not required.
