About the position

The Palliative RN in Advanced Illness Management at Illumia Health is responsible for providing high-quality end-of-life care to patients and their families. This role involves conducting in-person and telephonic assessments, coordinating care with an interdisciplinary team, and ensuring that patients receive the necessary support and education regarding their conditions. The position emphasizes compassionate care and collaboration with healthcare professionals to enhance the quality of life for patients facing advanced illnesses.

Responsibilities

  • Conduct in-person/telephonic RN assessments/screenings and provide education to palliative care patients, family members, or caregivers as appropriate.
  • Complete Risk Stratification Assessment to identify patient support needs per established protocols in consultation with the NP.
  • Observe patient visit frequency per individual need and risk identification.
  • Provide reliable education and instructions to patients and families with excellent customer service.
  • Consult with and educate patients/families regarding disease processes, self-care techniques, end-of-life care, medications, nutrition, and dietary needs.
  • Communicate findings that identify appropriate preventive and rehabilitative skilled nursing or therapy needs to the PCP or AIM Palliative NP.
  • Educate patients and families on medication administration and treatments as prescribed by the nurse practitioner or physician.
  • Coordinate care with palliative team members.
  • Communicate changes in condition or signs and symptoms of decline to Palliative NP and/or PCP.
  • Support hospice referral as appropriate.
  • Document timely clinical records on all patients including screening/assessment findings, physician orders, progress notes, and care plans.
  • Support care plan development and implementation for palliative care from the initial assessment through discharge.
  • Collaborate on development and modifications to the palliative plan of care, individualized for each patient.
  • Facilitate care for patients when a need is identified in accordance with the Plan of Care.
  • Participate in evening/weekend call as required, conducting on-call services competently.
  • Facilitate prescription transmission to pharmacy including controlled substances as delegated and licensed.
  • Communicate clinical findings to the nurse practitioner and/or PCP as needed, no less than bi-weekly.
  • Re-assess effectiveness of care plan regularly and modify as needed.
  • Maintain effective working relationships with supportive care team members and involve them when necessary.
  • Establish and maintain effective working relationships with those contacted in the course of work.
  • Communicate and document clinical findings, treatment plans, and care provided in the patient's medical record.

Requirements

  • Registered Nurse with current RN license to practice in the state.
  • 2 years of prior skilled Registered Nurse experience.
  • Demonstrated ability to provide quality patient care.
  • Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
  • Fluent competence in Microsoft Outlook, Word, and Excel.
  • Fluent in electronic medical record documentation.

Nice-to-haves

  • Experience in hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing.
  • Strong compassionate phone skills.
  • Good problem-solving skills.
  • Good time management skills.
