About the position

The Paraprofessional II/Case Manager position at Hale Na`au Pono is responsible for delivering Intensive in-Home and Intensive Independent Living Skills services to children experiencing serious emotional and behavioral disorders. This role involves working closely with a multi-disciplinary team to provide case management services, ensuring that children receive the necessary support to maintain their home and community connections. The position requires flexibility in working hours and a commitment to the program's mission of providing therapeutic family-based alternatives to institutionalization.

Responsibilities

  • Submit charges tags for services provided within one working day.
  • Receive assignments for the week and/or by the day from the Program Director/Clinical Director.
  • Maintain confidentiality of the persons served and adhere to an ethical code of conduct at all times.
  • Comply with current policies for health and safety procedures at all times.
  • Participate as a member of a multi-disciplinary team and implement prescribed services.
  • Provide one-to-one companionship skills training as scheduled with assigned persons served.
  • Conduct services primarily in the home or community, including transportation when necessary.
  • Assist with the admissions process by helping persons served complete necessary documentation.
  • Collaborate with other service providers for joint services planning.
  • Respect the cultural values and beliefs of persons served.
  • Document applicable information in progress notes within one working day.
  • Maintain case records according to policy and ensure face-to-face contact as per treatment plan.
  • Complete monthly case management reports for assigned persons served.
  • Serve on-call 24/7 and provide crisis intervention and stabilization services as assigned.
  • Participate in Treatment Team meetings with the child, family, and other team members.
  • Support the implementation of tasks defined in the Mental Health Treatment Plan under supervision.
  • Act as a consultant and coach with assigned families, providing support in various settings.
  • Intervene with families using strength-based approaches and develop necessary parenting and behavior management skills.

Requirements

  • Bachelor's degree in Social Work (BSW) and 2 years of experience, or a high school diploma/GED with at least 4 years of case management experience, preferably with children experiencing serious emotional disorders.
  • Knowledge of the culture and lifestyle of the community served.
  • Ability to interpret problems within a cultural context and apply cultural concepts in interventions.
  • Personal qualities of warmth, acceptance, sound judgment, emotional stability, and reliability.
  • A desire to work with children experiencing serious emotional disorders.
  • Valid driver's license and clean driving record within the past three years.

Nice-to-haves

  • Experience in providing crisis intervention and stabilization services.
  • Familiarity with mental health treatment plans and interdisciplinary team collaboration.
