About the position

As a Chaplain at Transitions Care, you will provide non-denominational spiritual care services to patients, families, and caregivers, ensuring their spiritual needs are met during hospice care. This role involves assessing spiritual needs, coordinating with other spiritual counselors, and participating in interdisciplinary care planning. You will also conduct memorial services and support the hospice philosophy of care.

Responsibilities

  • Assess spiritual needs of patients/families/caregivers in a timely manner.
  • Provide consultation to patients, families, caregivers, and team members in a non-denominational manner.
  • Contact patient's/family's preferred spiritual advisor for ongoing involvement.
  • Develop and document a plan for spiritual care in conjunction with the interdisciplinary team.
  • Evaluate and revise the plan for spiritual care on an ongoing basis.
  • Provide consultation, education, and support to the interdisciplinary team on spiritual care.
  • Establish networks with faith communities to teach about hospice philosophy.
  • Provide funeral or memorial services for patients as requested.
  • Conduct memorial services in nursing homes as requested.
  • Conduct at least one memorial service per year for family members of the bereavement program.
  • Maintain records in compliance with local, state, and federal laws.
  • Participate in evening/weekend on-call as required.
  • Participate in staff meetings and other related activities.
  • Interact positively with all staff to support the Company's mission.

Requirements

  • Graduate of accredited seminary or school of theology (B.D., M.Div. or equivalent theological degree), or degree in counseling, or equivalent experience.
  • Experience working with death and dying, individual/family/caregiver.
  • Ability to work as a member of an interdisciplinary team.
  • Comfort in an interfaith setting.
  • Ability to accept different lifestyles, cultures, beliefs, and values.
  • Ability to network with community clergy and congregations.
  • Knowledge of and commitment to Hospice philosophy of care.
  • Hospice/home health experience preferred.
  • Compliance with accepted professional standards and practices.
  • Demonstrate excellent observation and communication skills.
  • Self-directed with the ability to work with little supervision.
  • Flexible and cooperative in fulfilling all obligations.
