About the position

The Pathway of Hope Case Manager at The Salvation Army is a part-time role focused on providing comprehensive support to individuals and families experiencing poverty and homelessness. The position aims to break the cycle of generational poverty through assessment, planning, and implementation of individualized action plans that promote self-sufficiency and enhance client satisfaction. The Case Manager will collaborate with community partners and utilize various methodologies, including Strength's Perspective and Motivational Interviewing, to address the diverse needs of clients.

Responsibilities

  • Conduct intake assessments with new clients.
  • Participate in all trainings for Pathway of Hope & WellSky Community Services.
  • Complete all data entry requirements for WellSky Community Services.
  • Develop individualized action plans and set goals for each client.
  • Provide ongoing assessments for clients at 30, 60, 90 days, and 12 months.
  • Provide ongoing case management and referrals to clients on housing, health benefits, and financial support.
  • Maintain a current file of appropriate community resources for staff use.
  • Network with outside agencies and build relationships with community partners.
  • Compile monthly statistical reports as required by the Division.
  • Attend monthly case management meetings to update the team on service plan progress and emerging needs.

Requirements

  • Bachelor of Arts in Social Work preferred or related field.
  • Minimum of 2-3 years of Case Management experience.
  • Minimum of one year of experience working with low-income and/or homeless individuals and families.
  • Ability to pass a criminal background check.

Nice-to-haves

  • Knowledge of the unique issues of homelessness.
  • Basic understanding of mental health disorders.
  • Proficient in Microsoft Windows and use of Word and Excel.
