About the position

The Pathways Team Lead Case Manager at Columbia River Mental Health Services is responsible for coordinating care for clients with mental illness, connecting them to essential health and social services to promote positive health outcomes. This role involves working collaboratively with clients to identify recovery goals, creating functional plans, and providing education and support to enhance independence and recovery. The position emphasizes community-based care coordination and requires knowledge of local resources.

Responsibilities

  • Coordinate care to connect clients to meaningful health and social services.
  • Work collaboratively with clients to identify individual recovery goals.
  • Create functional plans to meet recovery goals through tracking appointments and building skills.
  • Teach and assist clients to improve living, coping, symptom management, communication, and interpersonal skills.
  • Collaborate and coordinate care with other treatment providers and community resources.
  • Provide assistance with crises and help develop crisis plans.
  • Help clients connect to community health care providers and assist with medical and dental appointments.
  • Advocate for clients' rights and fair treatment in all areas of life.
  • Maintain required paperwork and charting as per agency contracts.

Requirements

  • Bachelor's degree in Mental Health, Social Work, Psychology, or Human Services.
  • Approximately 1-2 years of experience in the social service field with mental illness or chemical dependency preferred.
  • Strong communication skills.

Nice-to-haves

  • Experience in community-based care coordination.
  • Knowledge of local resources including housing, Social Security, Medicaid, and Medicare.
