About the position

The Patient Assessment Coordinator at PAM Health is responsible for coordinating the process of providing accurate and timely data related to patient assessments, particularly for Medicare patients. This role involves overseeing data collection, ensuring compliance with CMS guidelines, and providing education to staff. The Coordinator plays a crucial role in the completion of the IRF/PAI forms and acts as a resource for problem-solving related to Medicare's payment systems.

Responsibilities

  • Coordinate the process of providing accurate and timely data for the Prospective Payment System and quality reporting program for Medicare patients.
  • Oversee all aspects of data collection on the IRF/PAI and ensure supporting documentation is present.
  • Provide staff education to ensure compliance with CMS guidelines.
  • Review each IRF/PAI form completely, accurately, and timely.
  • Monitor the accuracy in scoring functional status and patient care needs.
  • Input appropriate information for the IRF/PAI into the eRehabData system.
  • Coordinate the collection of patient information from medical records and complete assigned sections of the IRF/PAI form.
  • Set completion dates for the multi-disciplinary team regarding IRF/PAI forms.
  • Develop and interpret policies and procedures related to patient assessments.
  • Act as the primary resource for problem-solving regarding Medicare's IRF prospective payment system.

Requirements

  • Current RN License (CRRN preferred)
  • 5 years of experience in patient assessment
  • CPR Certification
  • BLS Certification
  • Associate of Science in Nursing (ASN)
  • Bachelor's degree in nursing preferred
  • Master's degree required for OT/PT/SLP
  • Experience in an acute hospital, rehabilitation, or LTACH setting

Nice-to-haves

  • Experience with Medicare's IRF prospective payment system
  • Knowledge of FIM scoring and impairment group coding
