About The Position

The Patient Care Coordinator Manager at Avera is responsible for overseeing the strategic growth plan for Avera@Home services, ensuring effective management of the Patient Care Coordinator (PCC) staff. This role involves collaboration with physicians, facilities, and clinic staff to facilitate smooth transitions for patients from acute care settings to home healthcare, hospice, and home medical equipment services. The position emphasizes teamwork, compassion, and innovation in advancing healthcare services.

Requirements

  • Bachelor's degree in nursing preferred.
  • Graduation from a CCNE or NLN accredited nursing program is strongly preferred.
  • Demonstrated sound communication and sales skills.
  • Training and experience in healthcare, home health, or hospice preferred.
  • Previous supervisory or management experience preferred.
  • Licensed as a Registered Nurse in the State of South Dakota.
  • Valid driver's license and insurable by Avera Health automobile liability insurance.

Nice To Haves

  • Licensed as a Registered Nurse in the State of Minnesota may be required depending on the service area.
