About the position

The Patient Care Coordinator position is a vital role within our organization, which has been dedicated to providing exceptional homecare and hospice services for over 35 years. As a Patient Care Coordinator, you will serve as a liaison between our services and the families we support, ensuring they understand the full range of home health and hospice options available to them. Your role will involve direct interactions with families, educating them about our services, and fostering referral relationships through networking and community engagement. In this position, you will be expected to conduct one-on-one contacts, participate in group presentations, and attend community events or trade shows to promote our services. You will also be responsible for tracking accounts and strategizing on how to build confidence with potential clients. Additionally, you will assist our staff in obtaining necessary physician signatures for orders and liaise with hospitals, physician offices, skilled nursing, and assisted living facilities to ensure seamless communication and care coordination. This role requires a motivated self-starter who is dependable, possesses integrity, and has a strong understanding of the local area and community relations.

Responsibilities

  • Conduct one-on-one contacts with potential clients and families.
  • Participate in occasional group presentations and community events or trade shows.
  • Educate families about home health and hospice services.
  • Track accounts and develop strategies to gain client confidence.
  • Assist staff in obtaining physician signatures for orders as needed.
  • Liaise with hospitals, physician offices, skilled nursing, and assisted living facilities.

Requirements

  • Experience in home health and hospice or a related field is required.
  • Community relations work experience is preferred.
  • Hospital/Physician marketing experience is a plus.
  • A valid Utah driver's license is required.
  • Dependable automobile is necessary.

Nice-to-haves

  • Clinical license is a plus but not required.
  • Customer service experience of at least 1 year is preferred.
