About the position

The Patient Care Coordinator at SMA Healthcare is responsible for managing inbound calls related to mental health and substance abuse services. This role requires proficiency in multiple electronic medical record platforms to document and track patient information. The coordinator will also engage with patients through telehealth services, gather necessary patient information, and respond to requests from hospital emergency departments. The position involves collaboration with team members and participation in mandatory training and meetings.

Responsibilities

  • Answer inbound calls from callers inquiring about service related to mental health and substance abuse.
  • Use established phone etiquette to answer and manage customer calls.
  • Work in multiple electronic medical record platforms to record and track information.
  • Answer phones, transfer calls, send emails, and respond to messages.
  • Gather patient information and complete documentation for services.
  • Respond to requests from hospital emergency department(s).
  • Use web cameras/video conference/telehealth with patients to provide information and education.
  • Work collaboratively in a group/team setting.
  • Serve as support to other team members.
  • Complete required SMA in-service trainings to maintain employment.
  • Attend mandatory meetings.
  • Perform miscellaneous job-related duties as assigned.

Requirements

  • High School Diploma and two (2) years of direct care experience working in Human Services or a related field.
  • Bachelor's degree in Human Services or related field and/or Certified Addictions Professional (CAP) preferred.
  • Knowledgeable in current best practices of behavioral health treatment and related services.
  • Knowledge of typical diagnoses, treatment services, and developmental issues for adolescents and adults.
  • Knowledge of clinical terms related to mental health and substance abuse.
  • Knowledge of the Baker Act and the Marchman Act.
  • Ability to perform solution focused and problem solving techniques.
  • Ability to multitask.
  • Proficiency in working with computer hardware and software.
