About the position

HarmonyCares is one of the nation's largest home-based primary care practices, dedicated to providing high-quality, coordinated health care in the home. The Patient Care Coordinator provides clinical support to HarmonyCares Medical Group patients primarily through phone encounters, answering inbound inquiries, triaging, and communicating with providers. This role is essential in assisting with care coordination efforts and proactively monitoring patients as instructed.

Responsibilities

  • Provide medical triage for inbound clinical inquiries via telephone calls and patient portal messages.
  • Maintain a paneled caseload of lower acuity patients, responsible for care plans and proactive outreach.
  • Initiate the SBAR process and utilize appropriate triage protocols when assessing clinical situations.
  • Document all phone calls and patient interactions in the EHR promptly.
  • Review, transcribe, communicate, and process providers' verbal orders as requested.
  • Assist with patients' medication refill requests, checking for duplicates and preparing necessary information for provider review.

Requirements

  • Ability to react decisively and quickly in emergencies.
  • Knowledge of medications effects, indications, and contraindications.
  • Experience with multi-tasking.
  • Proficiency in using electronic health records (EHR) system, call center technology, and Microsoft Office Suite.
  • Ability to communicate effectively with patients and their family members, and facility staff.
  • Knowledge of basic principles, practices and techniques in primary healthcare.
  • Ability to read and interpret written information, and write clearly and informatively.
  • Ability to speak clearly and persuasively in positive or negative situations.

Nice-to-haves

  • Active LPN/LVN license in the state where patients are located, Foreign Nursing Grad, or Foreign Medical Grad.
  • 2 years of experience as an LPN or LVN.
  • Familiarity with Medicare and Medicaid insurances and programs.
  • Experience in Aprima (EHR).
