About the position

The Patient Care Coordinator plays a crucial role in managing clinical records and ensuring compliance with state and federal regulations. This position is responsible for maintaining accurate patient records for clinical and billing purposes, while also performing various clerical duties to support the healthcare team in Minden, LA.

Responsibilities

  • Ensures clinical record systems are maintained in compliance with state and federal regulations.
  • Protects all clinical records including confidentiality of patient and organization information through the establishment and implementation of control procedures for all open and closed records.
  • Prepares all documentation for weekly IDT meetings including IDT agenda and all other supplemental paperwork.
  • Prepares copies of appropriate documentation for nursing facilities and home charts.
  • Completes an administrative record audit following patient discharge and reports findings to the Director of Clinical Services.
  • Monitors aged unsigned physician orders and submits requests for signed orders to overdue physicians.
  • Calls or visits the offices of seriously overdue physicians to obtain necessary documents required by state/federal regulations for medical records to ensure compliance for billing.
  • Communicates effectively and professionally with all stakeholders.
  • Manages/assists with incoming calls and faxes, and welcomes and assists all guests.
  • Performs other necessary functions/duties as assigned by the Director of Clinical Services and/or Administrator.

Requirements

  • An Accredited Records Technician Degree; Bachelor's degree preferred but not required.
  • At least two years' experience in clinical records management, preferably in hospice care operations.
  • Proficiency in Microsoft Office.
  • Demonstrated ability to multi-task effectively.
  • Knowledge of corporate business management and federal/state governmental regulations.
  • Excellent communication, grammar, and public relations skills.
  • Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
  • Self-motivated and a team player.
