About the position

The Patient Care Coordinator at Kidney Care Center is responsible for supporting various patient care programs, ensuring eligible patients are enrolled and effectively communicating between corporate and clinic teams. This role requires strong listening and communication skills to assess patient needs and provide appropriate care guidance, particularly in managing hypertension.

Responsibilities

  • Assist patients in managing their blood pressure through daily monitoring and calls.
  • Follow up monthly with assigned patients via phone to review daily blood pressure readings and educate on hypertension control.
  • Verify and record patient demographics, medical issues, medications, and risk management factors to create clinical summaries using specialized medical software.
  • Contact patients when blood pressure exceeds established parameters to evaluate the situation.
  • Communicate with practices and/or providers when readings exceed established parameters based on established protocols.
  • Help patients make timely contact with healthcare practitioners for urgent needs regarding hypertension.
  • Register chronic care management and principal care management.
  • Comply with documentation requirements of CCM and PCM programs by executing care plans and recording in the EMR.
  • Monitor adherence to care plans, evaluate effectiveness, and facilitate changes as needed.
  • Provide patient health counseling, education, and instruction.
  • Handle potentially difficult conversations calmly and professionally.
  • Participate in individual and group projects as directed by management.
  • Complete audits of Chronic Kidney Disease Patients to ensure appointments are scheduled.

Requirements

  • High school diploma or equivalent required.
  • Certified Medical Assistant certification is required.
  • 2+ years of experience with Electronic Health Records (EHR).
  • Previous call experience with U.S. based clients.
  • Ability to multitask and handle multiple phone lines without assistance.
  • Proficient in Microsoft Office Suite: Outlook, Word, and Excel.

Nice-to-haves

  • Associates' degree preferred.
