About the position

The Patient Care Coordinator at UnitedHealth Group plays a crucial role in ensuring the smooth operation of patient care processes and protocols. This position is integral to the care team, working collaboratively to support the organization's vision and mission while delivering exceptional customer service. The coordinator is responsible for adhering to Lean processes and supporting the team in achieving financial, clinical, and service goals. The role involves a variety of tasks, including managing adherence to quality measures, ensuring accurate patient demographics, scheduling appointments, and working closely with healthcare providers to ensure that all necessary care measures are completed for the Medicare population. The coordinator also assists in resolving service issues and complaints, ensuring a seamless patient experience, and may be required to assist co-workers in various capacities as needed. The work schedule for this position is Monday through Friday, from 7:30 AM to 5:00 PM, with the supervisor determining the specific schedule within these hours. The position is located at 1580 W Antelope Dr. Suite 200, Layton, UT. Responsibilities

  • Provide exceptional service to all customers.
  • Responsible for adherence management for quality measures.
  • Ensure demographics including insurance information are correct in scheduling and practice management software, along with updates as necessary per Standard Work.
  • Schedule appointments, verifying correct visit type and Primary Care Provider empanelment.
  • Work closely with Providers on care for Medicare population to ensure all Medication, HEDIS measures, and Annual Wellness Exams are completed each year.
  • Work with back-office staff to ensure smooth patient flow.
  • Resolve service issues and complaints and defer to appropriate management or clinical personnel when necessary.
  • Assist co-workers and team members with duties when requested, to include but not limited to, floating to other areas.
  • Perform other duties as assigned.

Requirements

  • High School Diploma/GED (or higher)
  • 1+ years of customer service experience
  • 6+ months of experience in a healthcare office setting
  • Intermediate level of proficiency in Microsoft Office

Nice-to-haves

  • Ability to do mathematical computations for Medication Adherence
  • Experience handling high volume of telephone calls and a multi-system phone line
  • Ability to navigate Medicare portal to verify patient eligibility and benefits
  • 1 + years of experience with knowledge of Medication terminology
  • 1+ years of experience with knowledge of Medical terminology
