About the position

The Patient Care Coordinator at Vista Center for the Blind and Visually Impaired is responsible for providing exceptional support to patients through direct interaction, efficient documentation, and effective communication. This role is crucial in guiding patients through their journey with the center, ensuring they receive necessary services and support in a compassionate and organized manner.

Responsibilities

  • Serve as the primary point of contact for patients, providing information about services, scheduling appointments, and answering questions with empathy and clarity.
  • Work closely with patients, families, and clinical staff to coordinate appointments, follow-ups, and service plans.
  • Maintain accurate patient records in the EHR system and database, ensuring all documentation is up-to-date and compliant with privacy regulations.
  • Facilitate clear and effective communication between patients, their families, and Vista Center's multidisciplinary team to provide seamless care.
  • Compile and analyze data related to patient services, outcomes, and satisfaction to support reporting and continuous improvement initiatives.
  • Assist with patient referrals, external resources, and coordination of community support services as needed.
  • Perform general administrative duties, such as managing incoming calls, emails, and completing patient orders.

Requirements

  • High school diploma or equivalent required; Associate's or Bachelor's degree in a related field is a plus.
  • Minimum of 2 years of experience in a healthcare or social services setting, preferably in patient coordination.
  • Excellent interpersonal and communication skills, both verbal and written.
  • Strong organizational skills with the ability to manage multiple tasks and priorities.
  • Proficiency in using patient management software, databases, and Microsoft Office Suite.
  • Knowledge of vision loss services or experience working with individuals with disabilities is highly desirable.
