About the position

The Patient Care Manager RN is responsible for overseeing clinical departments and ensuring the quality and safety of services delivered in compliance with state and federal regulations. This role involves supervising nursing staff, coordinating patient care, and providing education and support to both employees and patients. The position requires strong leadership skills and a commitment to enhancing the lives of patients with life-limiting illnesses.

Responsibilities

• Direct and coordinate clinical departments and sites, ensuring compliance with state and federal regulations.
• Supervise and provide direction to nursing staff to ensure quality and continuity of care.
• Support employee and patient education to enhance knowledge and skills.
• Continuously train nursing staff to maintain high-quality patient care.
• Provide coaching, development, feedback, and annual evaluations to team members.

Requirements

• Current State Registered Nurse License
• Current State Driver's License and reliable transportation
• Two years of experience as a Registered Nurse in a clinical care setting, preferably in hospice or home health
• Strong organizational, interpersonal, and communication skills
• Ability to work well in a multidisciplinary group setting and manage multiple projects.
