About the position
The Patient Care Manager RN at Heart of Hospice is responsible for leading and managing the delivery of exceptional patient care in a hospice setting. This role emphasizes a culture of caring, belonging, and trust, focusing on meaningful connections with patients, families, and the community. The position involves both office and field work, ensuring compliance with hospice policies and regulatory requirements while promoting quality care and safety for patients and staff.
Responsibilities
- Lead, inspire, and motivate staff to provide exceptional patient care.
- Ensure all staff are oriented and supported for successful job performance.
- Act as a liaison between staff, patients, and the medical community.
- Assist the Executive Administrator/Executive Director/Director of Clinical Operations with patient care activities.
- Ensure compliance with hospice policies and procedures, as well as local, state, and federal regulations.
- Promote and direct the assigned team regarding quality of care and patient safety.
- Engage in problem resolution and complaint investigation.
- Provide in-services to staff on health and safety, regulatory issues, and agency policies.
Requirements
- Current RN licensure in the state of practice.
- One year of clinical experience as an RN.
- Current CPR Certification.
- Current driver's license and valid vehicle insurance.
- Access to a dependable vehicle or public transportation.
- A minimum of two years full-time experience working as an RN in Louisiana, or two years of full-time clinical experience in hospice care as a licensed practical nurse may substitute for the RN experience.
