About the position

The Senior Patient Care Manager at SouthernCare New Beacon is responsible for overseeing the daily operations of a hospice site, ensuring high-quality patient care, and managing the performance of team members. This role involves direct patient care, administrative duties, and acting as a leader in the absence of the Executive Director or Administrator, all while maintaining a compassionate approach to terminally ill patients and their families.

Responsibilities

• Supervise, manage performance, and direct the work of specified team members.
• Assign admit and evaluation visits, schedule visits, and ensure timely completion of visits by staff.
• Manage the site in the absence of the Executive Director or Administrator.
• Oversee clinical, administrative, and recordkeeping functions.
• Respond to phone referrals/inquiries in a timely, professional, and compassionate manner.
• Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.

Requirements

• Degree in nursing; Registered Nurse with ASN or BSN from an accredited school of nursing.
• One year of experience as an RN, licensed in the state of requested employment.
• Three years' experience in direct patient care, with one year of management experience.
• Two or more years' experience in hospice or home care.
• Ability to operate nursing assessment tools and office equipment, including computers and tablets.
• Sensitivity to the feelings of terminally ill patients and their families.

Nice-to-haves

• Knowledge of terminally ill patients and their families along with an understanding of the hospice concept.
• Excellent patient assessment skills.
• Good oral and written communication skills.
