About the position
The RN Patient Care Manager at Gentiva Hospice is responsible for overseeing the day-to-day operations of the assigned hospice site, ensuring the delivery of quality care to patients. This role involves managing clinical and administrative functions, responding to patient inquiries, and providing direct nursing care as needed. The position requires strong leadership skills and a compassionate approach to patient care, particularly for those facing terminal illnesses.
Responsibilities
- Manage the day-to-day operations of the assigned hospice site.
- Administer the clinical aspects of the hospice program.
- Ensure the provision of quality care to patients.
- Oversee clinical and administrative recordkeeping functions.
- Respond to phone referrals and inquiries in a timely and compassionate manner.
- Participate in the on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
Requirements
- Registered Nurse with a minimum of Associate's Degree in Nursing from an accredited school of nursing.
- Bachelor's Degree in Nursing preferred.
- Three years' experience in direct patient care in a clinical setting required.
- Minimum one year of management/supervisory experience required.
- Two or more years' experience in hospice or home care required.
- Current license to practice nursing in the state of requested employment.
- CPR Certification.
- Current automobile insurance and valid driver's license.
Nice-to-haves
- Experience with managed care principles and regulatory guidelines (i.e., Medicare, Medicaid).
- Excellent patient assessment skills.
- Good oral and written communication skills.
