About the position

The Patient Care Manager at Gentiva Hospice is responsible for overseeing the daily operations of the hospice site, ensuring the delivery of quality care to patients and their families. This role involves managing clinical and administrative functions, responding to patient inquiries, and providing direct nursing care as needed. The position requires strong leadership skills and a compassionate approach to patient care, particularly for those who are terminally ill.

Responsibilities

  • Manage the day-to-day operations of the assigned hospice site.
  • Administer the clinical aspects of the hospice program.
  • Ensure the provision of quality care to patients.
  • Oversee clinical and administrative recordkeeping functions.
  • Respond to phone referrals and inquiries in a timely and compassionate manner.
  • Participate in the on-call nursing schedule.
  • Provide direct skilled bedside nursing care in patients' homes.

Requirements

  • Registered Nurse with a minimum of Associate's Degree in Nursing from an accredited school of nursing.
  • Bachelor's Degree in Nursing preferred.
  • Three years' experience in direct patient care in a clinical setting.
  • Minimum one year of management/supervisory experience required.
  • Two or more years' experience in hospice or home care required.
  • Current license to practice nursing in the state of employment.
  • CPR Certification.
  • Current automobile insurance and valid driver's license.
