About the position
The Patient Care Manager at Gentiva Hospice is responsible for overseeing the daily operations of a hospice site, ensuring the delivery of high-quality clinical care to patients. This role involves managing clinical and administrative functions, responding to patient inquiries, and providing direct nursing care as needed. The Patient Care Manager plays a crucial role in supporting both patients and their families during challenging times, while also managing the site in the absence of higher-level management.
Responsibilities
- Manage the day-to-day operations of the assigned hospice site.
- Administer the clinical aspects of the hospice program.
- Ensure the provision of quality care to patients.
- Oversee clinical and administrative recordkeeping functions.
- Respond to phone referrals and inquiries in a timely and compassionate manner.
- Participate in the on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
Requirements
- Registered Nurse with a minimum of Associate's Degree in Nursing from an accredited school of nursing.
- Bachelor's Degree in Nursing preferred.
- Three years' experience in direct patient care in a clinical setting required.
- Minimum one year of management/supervisory experience required.
- Two or more years' experience in hospice or home care required.
- Current license to practice nursing in the state of requested employment.
- CPR Certification.
- Current automobile insurance and valid driver's license.
