About the position

The Patient Care Partner - Telehealth role at Lehigh Valley Health Network involves collaborating with clinical departments to facilitate telehealth encounters between patients and providers. This position aims to support patient treatment locally, reducing the need for interfacility transfers by enabling specialists to provide virtual care. The role includes direct patient care, utilizing technology for virtual encounters, and ensuring effective communication within the care team.

Responsibilities

  • Facilitates clinical exams under the direction of distant providers using required peripheral devices and technology.
  • Serves as a liaison between unit/practice staff and providers to coordinate virtual care encounters.
  • Provides patient care and related activities using a patient/family centered approach.
  • Communicates findings, changes, or problems to the RN effectively.
  • Supports unit-based patient experience goals and hospital objectives as a team member.
  • Participates in the orientation of new staff as assigned.
  • Performs validated technical skills based on clinical specialty and unit practice.
  • Completes quality control checks on designated unit equipment, if applicable.
  • Conducts patient rounds, anticipates patient needs, and responds promptly to patient calls.

Requirements

  • Must be 18 years old or older.
  • Six months recent experience in direct provision of patient care activities, including treatments and/or diagnostic studies, or successful completion of LVHN's inexperienced technical partner course.
  • Ability to follow direction and provide accurate reports.
  • Attainment and maintenance of unit-specific competencies.
  • Ability to organize and prioritize assignments in the delivery of patient care.
  • Basic computer skills.
  • Demonstrates ability to work with clinical leadership and staff level clinicians across various service lines.
  • American Heart Association Basic Life Support certification within 180 days.

Nice-to-haves

  • High School Diploma/GED or Associates/Bachelor's Degree in a healthcare-related field.
  • CMA - Certified Medical Assistant certification.
  • CNA - Certified Nursing Assistant certification.
