About the position

The position involves working collaboratively with Seaside Pediatrics providers and clinical support staff to promote coordination with outside providers where applicable, such as therapists, schools, social service agencies, and courts. The role includes locating outside specialty services for patients when needed and facilitating coordination of appointments and communication with healthcare staff and providers. The individual will assess and address any barriers to care, support patients and families in establishing recommended services, and assess patients' needs and eligibility for services. Additionally, the position requires facilitating referrals to specialists and community agencies, participating in patient care team meetings, and ensuring that the multidisciplinary treatment plan aligns with the patient's and family's needs. The individual must remain up to date on community resources, provide resource materials and education in the patient's primary language, and engage interpreter services when necessary. The role also includes performing other work-related duties as assigned and consistently providing service excellence to all patients, family members, visitors, volunteers, and co-workers.

Responsibilities

  • Supports patients and families by providing assistance in establishing recommended services.
  • Assesses patient's needs and eligibility for services.
  • Facilitates referrals to specialists and community agencies.
  • Participates in patient care team meetings to assist in determining appropriate patient care services.
  • Ensures that the multidisciplinary treatment plan is consistent with the patient's/family's needs.
  • Remains up to date on resources available within the community.
  • Receives feedback from patients to verify helpfulness or identify concerns.
  • Ensures all resource materials are updated as needed.
  • Provides resource materials and education at an appropriate education level.
  • Engages interpreter services when needed.
  • Utilizes interventions and strategies appropriate to the population.
  • Performs other work-related duties as assigned or requested.

Requirements

  • Possess a HS diploma.
  • Prior experience in a healthcare setting preferred.
  • Possess the ability to communicate orally and in writing.
  • Work effectively in a team environment.
  • Have excellent interpersonal skills.
