About the position

The position functions as part of the dialysis healthcare team, providing safe and effective home dialysis therapy and training for patients under the supervision of a licensed nurse. The role involves assisting in the setup of home dialysis equipment, maintaining a safe working environment, and supporting the organization's Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) activities. The PCT II HT is responsible for reinforcing training, obtaining vital signs, managing patient records, and ensuring compliance with regulations.

Responsibilities

  • Reinforces home hemodialysis (HHD) and/or peritoneal dialysis (PD) training as permitted by law.
  • Identifies and welcomes patients, obtains vital signs, weights, and gathers information needed to complete the Clinic Visit Intake form.
  • Builds and maintains strong communication and collaboration processes with patients, care partners, and team members.
  • Obtains treatment records from patients, reviews for completion, and reports missing or abnormal data to the HT RN.
  • Assists with electronic home treatment/medication reconciliation and reports missing data/orders to the HT RN.
  • May assist HT RNs with home visits and provides home support as applicable.
  • Prepares supplies for laboratory testing and assists with tracking and shipping of transplant-related specimens.
  • Completes daily chores and documents appropriately, disinfecting dialysis machines and equipment.
  • Prepares the exam/training room for home dialysis training and treatment visits.
  • Schedules patient appointments and contacts patients/caregivers regarding dates and reminders.
  • Maintains logs and tracking, and may order and track clinic supplies as directed.
  • Participates in medical records, infection control, and audit processes as directed.
  • Helps collect required documentation for patient admission and maintains patient charts as directed.
  • Performs clerical duties such as answering phones and faxing as assigned.
  • Sets up, tears down, programs, tests, and disinfects the HHD machine as directed.
  • Troubleshoots HHD machinery/tools and connectivity issues, and prepares dialysate for use as directed.
  • Initiates HHD treatment, monitoring and documenting patient status and vital signs during treatment.
  • Reports significant changes in patient status during treatment to the HT RN and documents in the medical record.
  • Reinforces PD training and troubleshoots PD cycler and connectivity issues.
  • Participates in the recruitment process for new clinical staff and assists with clinical orientation.

Requirements

  • High School diploma or G.E.D. required.
  • Valid PCT national certification or state-specific certification as defined by CMS.
  • At least two years of previous patient care experience in a hospital setting or related facility preferred.
  • Successful completion of the organization's dialysis training program and CPR certification.
