About the position

The Patient Engagement Specialist will play a crucial role in the Population Health Team at Oak Street Health, focusing on conducting outreach to patients who are not currently engaged in the clinic's care model. This position involves both telephonic and in-person home visits to foster patient engagement and support access to care, ensuring that patients receive the necessary services in a timely manner.

Responsibilities

  • Conduct patient outreach through telephonic and in-person home visits.
  • Engage with patients who have not visited the clinic to support their access to care.
  • Spend approximately half of the time in the community connecting with patients in various settings such as homes, hospitals, and long-term care facilities.
  • Respond to escalations and urgent needs, prioritizing work and multitasking effectively.
  • Document patient interactions in electronic medical records while maintaining HIPAA standards.
  • Communicate complex topics among all parties involved in patient care, including family members and medical providers.
  • Partner with patients and their support persons to identify goals and address barriers to care.
  • Foster relationships with patients and caregivers independently.
  • Collaborate with Community Health Workers, Social Workers, and clinicians to support complex patient needs.
  • Educate patients about Oak Street's wrap-around services.

Requirements

  • Experience in healthcare-related engagement, sales, or marketing.
  • Strong verbal and written communication skills.
  • Proficiency in technology, including Excel and EHR/EMR platforms.
  • Dedication to community service and building relationships.
  • Ability to operate independently in a fast-paced environment.
  • Understanding of Medicare and Medicare Advantage programs.
  • Access to reliable transportation for patient visits.
  • Fluency in Spanish (preferred).
  • US Work Authorization.
