About the position

CommonSpirit Health, formed by the alignment of Catholic Health Initiatives (CHI) and Dignity Health, is seeking a dedicated individual to coordinate transfer center activities for Dignity Health facilities. This position is based onsite at our Rancho Cordova, CA office on Data Drive. The incumbent will serve as the primary liaison between referring hospitals and Dignity Health facilities, ensuring the smooth transfer and admission of patients. This role requires collaboration with hospital administration, nursing leadership, medical staff, physicians, and transportation vendors to facilitate patient transfers effectively. The incumbent will participate in all activities, meetings, calls, and educational sessions to prioritize bed assignments from various entry points into the acute hospital setting. The role involves making critical decisions regarding facility designation based on patient diagnoses, planned procedures, and care requirements. The incumbent will collect both objective and subjective data to ensure appropriate patient transfers and maintain constant communication with internal and external sources regarding patient transfers, utilizing proper etiquette. In emergency situations, the incumbent must respond efficiently to ensure the appropriate transfer of all patients, making judgments and modifying transfer priorities based on evaluations. This includes timely reporting of any abnormal changes in patient conditions to the appropriate personnel. The role also requires proficient and competent computerized documentation, adherence to policies and procedures, and accountability for decisions and behaviors. The incumbent will set measurable written goals during the annual appraisal process and demonstrate efforts to achieve these goals throughout the year, all while maintaining a safe working environment by utilizing appropriate resources and protocols.

Responsibilities

  • Coordinate transfer center activities for Dignity Health facilities in collaboration with hospital administration, nursing leadership, medical staff, physicians, and transportation vendors.
  • Serve as the primary liaison between referring hospitals and Dignity Health facilities to coordinate patient transfers and admissions.
  • Participate in activities, meetings, calls, and education to facilitate and prioritize bed assignments from all points of entry to the acute hospital setting.
  • Collect objective and subjective data for effective and appropriate patient transfers.
  • Determine facility designation based on diagnoses, planned procedures, and level of care requirements.
  • Maintain constant communication with internal and external sources regarding patient transfers using proper etiquette.
  • Utilize nursing knowledge and assessment to prioritize patient transfers based on urgency of condition and available beds.
  • Communicate effectively with hospital staff to facilitate patient flow.
  • Respond efficiently during emergency situations for appropriate patient transfers.
  • Make judgments and decisions to modify patient transfer priorities based on evaluations and report any changes in patient conditions in a timely manner.
  • Perform legible, timely, concise, and accurate documentation according to policies and procedures.
  • Demonstrate proficiency in computerized documentation and accept responsibility for decisions and behaviors.
  • Recognize and cooperate with direction from facility leadership.

Requirements

  • Current California LVN License
  • 2 years of clinical experience in a healthcare setting
  • Knowledge of current nursing techniques, principles, and practices
  • Working knowledge of computer programs such as Gmail/Google professional suite
  • Ability to navigate hospital software and possess typing skills
