About the position

The Patient Logistics LVN at Dignity Health is responsible for coordinating transfer center activities for Dignity Health facilities. This role involves acting as the primary liaison between referring hospitals and Dignity Health facilities to ensure effective patient transfers and admissions. The incumbent will utilize nursing knowledge to prioritize patient transfers based on urgency and maintain communication with various stakeholders to facilitate patient flow.

Responsibilities

  • Coordinate transfer center activities for Dignity Health facilities in collaboration with hospital administration, nursing leadership, medical staff, and transportation vendors.
  • Act as the primary liaison between referring hospitals and Dignity Health facilities to coordinate patient transfers and admissions.
  • Participate in activities, meetings, and education to facilitate bed assignments from all points of entry to the acute hospital setting.
  • Collect objective and subjective data for effective patient transfers.
  • Determine facility designation based on diagnoses, planned procedures, and level of care requirements.
  • Maintain constant communication with internal and external sources regarding patient transfers.
  • Utilize nursing knowledge to prioritize patient transfers based on urgency and available beds.
  • Communicate effectively with hospital staff to ensure smooth patient flow.
  • Respond efficiently during emergencies for appropriate patient transfers.
  • Make judgments and modify patient transfer priorities based on evaluations.
  • Document patient conditions accurately and timely according to policies and procedures.
  • Accept responsibility for decisions and behaviors, and cooperate with facility leadership.
  • Set measurable written goals during annual appraisals and work towards achieving them throughout the year.
  • Maintain a safe working environment by following protocols and communicating with appropriate personnel.

Requirements

  • Current California LVN License
  • 2 years of clinical experience in a healthcare setting
  • Knowledge of current nursing techniques, principles, and practices
  • Working knowledge of computer programs such as Gmail/Google suite
  • Ability to navigate hospital software and possess typing skills
