About the position

The Patient Logistics Nurse at CommonSpirit Health plays a crucial role in coordinating patient transfers and admissions to Dignity Health facilities. This position is part-time, with a mid-shift schedule from 3:00 PM to 3:00 AM, requiring two shifts per week, which may include weekends. The nurse will work closely with hospital administration, nursing leadership, medical staff, and transportation vendors to ensure smooth transitions for patients from referring hospitals into Dignity Health facilities. The Patient Logistics RN will assess facility designations based on patient diagnoses, planned procedures, and care requirements, ensuring that patients receive the appropriate level of care based on their acuity. Additionally, the nurse will participate in various activities, meetings, and educational sessions to facilitate patient transfers from all points of entry into the acute hospital setting. Responsibilities

  • Coordinate transfer center activities for Dignity Health facilities.
  • Act as a liaison between referring hospitals and Dignity Health facilities for patient transfers and admissions.
  • Assess facility designations based on patient diagnoses and care requirements.
  • Participate in activities, meetings, and education to facilitate patient transfers.
  • Collaborate with hospital administration, nursing leadership, medical staff, and transportation vendors.

Requirements

  • One year of recent acute care experience required.
  • RN with three years of clinical experience in a hospital setting, including at least one year in emergency and/or critical care.
  • ADN, Diploma, or foreign equivalent required.
  • Current AZ State RN license in good standing required.
  • Competent in select psychomotor skills and knowledgeable in current nursing techniques, principles, and practices.
  • Basic computer skills and working knowledge of programs such as Word, Excel, and Outlook.

Nice-to-haves

  • Background in Utilization Management and knowledge of healthcare reimbursement.
  • Experience in Neurology/Neurosurgical and Cardiology.
  • Experience using electronic bed management systems, preferably TeleTracking.
  • Experience in Patient Flow management.
  • Familiarity with Google Workspace.
