About the position

The Patient Navigator Coordinator at Willamette Valley Cancer Institute & Research Center plays a crucial role in guiding patients through the cancer care continuum. This position is designed to enhance the patient experience by providing support, education, and coordination of care among various healthcare providers. The coordinator acts as a liaison for patients, families, and caregivers, ensuring that their needs are met throughout their treatment journey.

Responsibilities

  • Coordinate patient care through all phases of the care continuum under the supervision of the Support Services Supervisor.
  • Educate patients, families, and caregivers about the role of the Patient Navigation Coordinator.
  • Facilitate communication among healthcare providers.
  • Support patients and families with the completion of medical forms.
  • Coordinate necessary clinical appointments and convey information about resource availability.
  • Report patient and family challenges to the Multidisciplinary team and assist with potential solutions.
  • Ensure closure of all referral loops and document all patient interactions in the electronic medical record.
  • Act as a liaison and advocate for patients, families, and caregivers.
  • Participate in continuous process improvement of patient outcomes and resource utilization.
  • Maintain patient privacy according to local and national laws.
  • Participate in local and US Oncology Network-sponsored events.

Requirements

  • High school diploma or GED; advanced education preferred.
  • Minimum four (4) years of experience in a clinical healthcare environment.
  • Medical Assistant (certified) experience with oncology patient population preferred.
  • Oncology-related education or similar experience preferred.
  • Strong organizational and interpersonal skills.
  • Proficient use of an electronic medical record (EMR) system.
  • Strong knowledge of HIPAA compliance and patient confidentiality regulations.
  • Ability to prioritize and re-prioritize quickly.
  • Strong written and verbal communication skills, including medical terminology.

Nice-to-haves

  • Strong customer service skills.
  • Knowledge of oncology patient care and outpatient practice workflow.
  • Intermediate computer skills, including MS Office applications.
