About the position

The Patient Navigator/Data Analyst position is a full-time role focused on supporting a CDC-funded program aimed at reducing HIV and other communicable diseases. The role involves a combination of case management, data analysis, and direct client interaction to facilitate HIV testing and care linkage, while also contributing to programmatic quality improvement efforts under physician supervision.

Responsibilities

  • Identify candidates for routine HIV testing in the adult emergency department.
  • Interact with clients to perform and document rapid, point-of-care HIV testing according to hospital and state guidelines.
  • Provide appropriate pre- and post-counseling and education to clients receiving rapid HIV tests.
  • Link newly diagnosed HIV+ clients to care in designated HIV clinics for immediate antiviral treatment.
  • Connect eligible patients to appropriate clinical outpatient care, including those with newly diagnosed HIV+, out of care HIV+, and sexually transmitted diseases.
  • Provide counseling and education for HIV- clients regarding PrEP and PEP.
  • Document patient interactions and communications in the EMR per program protocols.
  • Participate in quality improvement efforts and focus group interviews for program enhancement.
  • Attend job-related education training, seminars, and conferences as determined by the program manager.
  • Identify internal and external sources of clinical data and conduct literature searches of peer-reviewed publications.
  • Prepare tables, graphs, fact sheets, and written reports summarizing research results.
  • Manage databases and perform descriptive and multivariate statistical analyses of data using computer software.

Requirements

  • College degree required; Master's preferred in relevant fields such as Healthcare, Public Health, or Epidemiology.
  • Strong understanding of medical terminology and clinical workflows.
  • Experience with patients living with HIV preferred.
  • Exceptional attention to detail and accuracy.

  • Bilingual in Spanish/English preferred.
