About the position

The Patient Navigator position at Mercy involves providing coordinated and compassionate service to patients and their families within the vAcute program. This role is essential in facilitating patient experiences by answering calls, directing patients to appropriate care, and collaborating with healthcare professionals to optimize patient outcomes. The position requires working night shifts and includes rotating weekends and holidays.

Responsibilities

  • Facilitate patient and family experience at Mercy Virtual by providing coordinated service.
  • Answer telephone calls from patients and families in the Mercy Virtual vAcute Program.
  • Collaborate with the patient's primary care physician and specialists to ensure appropriate care.
  • Utilize technology for on-demand video visits and triage tools to assist patients.
  • Perform duties in alignment with Mercy's mission, values, and service standards.

Requirements

  • Previous experience in a Medical Office, Emergency Department, or Hospital.
  • High school diploma or GED required.
  • BLS certification required or to be obtained within 6 months of hire.
  • Strong critical thinking and problem-solving skills.
  • Professional verbal and written communication skills.
