About the position

The Patient Navigator is a part-time role focused on supporting patients enrolled in a research project aimed at improving health during pregnancy and postpartum. This position serves as a liaison between patients and their case management team, providing education and resources to enhance patient experiences and outcomes. The ideal candidate will have a passion for helping others and the ability to communicate effectively in both English and Spanish.

Responsibilities

• Identifies eligible patients.
• Assists patients with gathering information for application.
• Collects metrics and data on participating patients.
• Accompanies and supports patients through the transition process.
• Provides resources to appropriate support groups for patients and families.
• Coordinates scheduling appointments, referrals, transportation, and other needed services.
• Monitors and documents appropriate transfer of patient information.
• Creates Standard Operating Procedures (SOPs) and recommends process improvements.
• Attends and contributes to work-related meetings.
• Collaborates with patients, providers, and staff.
• Performs other job duties as assigned.

Requirements

• High School diploma or GED.
• Four years of relevant experience.

• Bilingual in English/Spanish.
