About the position

The Patient Navigator (Part-Time) plays a crucial role in linking patients to community resources and coordinating care for high-risk patient populations with significant Social Determinants of Health (SDoH) needs. This position requires collaboration with clinical teams and office staff to identify and address the complex needs of patients, ensuring they receive the necessary support and resources for improved health outcomes.

Responsibilities

  • Learn about patients and their problems, inspiring confidence and developing trust and rapport.
  • Complete and interpret SDoH screening tools used by patients in clinical practice.
  • Connect with community-based organizations to develop a network of referral services related to SDoH.
  • Act as a liaison between patients, clinic operations, and clinical teams to address SDoH needs.
  • Facilitate communication with outside organizations to ensure proper linkages and continuity of care.
  • Analyze data from SDoH screening tools to identify population segments with specialized needs.
  • Run patient reports and review findings at regular team meetings.
  • Collect, analyze, and report patient SDoH experience feedback for process improvement.
  • Work cooperatively with all staff members and outside sources to deliver high service levels.
  • Observe and practice all VHT Patient Experience Service Standards during interactions with patients and staff.

Requirements

  • Minimum of 1 year experience in a healthcare setting.
  • High School Diploma or GED.
  • Understanding of community resources.
  • Willingness to connect with community-based organizations.
  • Average to advanced computer skills (Microsoft Word, Excel, Outlook).
  • Current Basic Life Support (BLS) certification.
  • Strong organizational skills and ability to multi-task.
  • Ability to work as a team member and independently.
  • Strong customer service skills, preferably in a service industry.
  • Effective verbal and written communication skills.
  • Strong data analysis skills.
  • Fluent in conversational English; knowledge of Punjabi or Spanish is preferred.
  • Proactive approach to providing a world-class patient experience.
  • Valid California Driver's License and proof of automobile insurance.

Nice-to-haves

  • Fluent in Spanish (required)
  • Experience in customer service (2 years preferred)
  • Experience in a clinical work setting (2 years required)
