About the position

The Patient Navigator RN will act as a liaison for patients diagnosed with cancer, facilitating their journey through the continuum of care at PMC. This role involves collaboration with various departments to ensure timely access to services and interventions, while also providing education and support to patients and their families throughout the treatment process.

Responsibilities

  • Develop relationships with physicians and departmental staff to assist with access issues and to assure timely diagnosis and testing.
  • Keep communication open with providers and patients to coordinate patient care services.
  • Develop relationships with the American Cancer Society, Kentucky Cancer Control, support groups and other agencies to make appropriate referrals.
  • Engage the patient and the patient's family to assure that they understand the process of cancer care.
  • Gather information and direct patients to websites to gain educational information.
  • Regularly communicate with the patient during active care and at appropriate intervals after the completion of active care.
  • Assure access to supportive care.
  • Work cooperatively as a member of the oncology care team.
  • Maintain documentation, algorithms and a tickler file to assure timely follow-up and alerts.
  • Act as a patient advocate.
  • Act as a community health educator.
  • Adhere to dress code.
  • Adhere to medical center policies addressing infection control, safety, and personnel guidelines.
  • Demonstrate knowledge of and compliance with the medical center code of conduct and applicable corporate policies and procedures.
  • Maintain patient confidentiality and treat patients and their families with respect and dignity.
  • Function as a preceptor to orient new employees.
  • Perform other related duties as assigned or requested.

Requirements

  • Interpersonal skills.
  • Experience with working with oncology patients and families is preferred.
  • Knowledge of the breadth and accessibility of community resources.
  • Thorough knowledge of health care systems and terminology.
  • Working knowledge of HIPAA regulations.
  • Self-directed and able to organize and manage multiple tasks/projects simultaneously.
  • Ability to promote programs and services to the community.
  • Extensive understanding of oncology care.
  • Proficient in learning medical record systems (electronic and non-electronic).
  • Written and verbal communication skills.

Nice-to-haves

  • Bachelor's Degree in Nursing is preferred.
  • Three or more years of experience, preferably with oncology patients.
