About the position

The Patient Navigator (RN) plays a crucial role in enhancing the patient experience by providing individualized support and guidance throughout complex treatment journeys. This position is responsible for coordinating care, ensuring effective communication between patients and healthcare providers, and facilitating a seamless experience during hospital stays or surgical visits.

Responsibilities

  • Coordinate PAT appointments and pre-op interviews for outpatient procedures.
  • Conduct pre-op phone calls with patients for nursing assessment and education.
  • Ensure patients understand their admission status, whether inpatient or same-day discharge.
  • Communicate with the Unit Charge Nurse regarding patient scheduling for surgery/procedure and coordinate bed availability.
  • Verify that the Primary Nurse has completed pre-op orders and initiate the OR Readiness Checklist.
  • Review labs, consents, and diagnostic tests, and communicate alerts to Anesthesia.
  • Visit patients at bedside to confirm readiness when necessary.
  • Communicate with the Charge Nurse regarding patient readiness and organize transport.
  • Notify the OR Charge Nurse when the patient is brought to pre-op holding.
  • Check patients in, verifying correct patient information and conducting a full body assessment.

Requirements

  • One year of experience in Perioperative Services.
  • Strong critical thinking and service excellence skills.
  • Good interpersonal communication skills.
  • Ability to read and comprehend written instructions.
  • Strong organizational skills and ability to follow verbal instructions.
  • Proficient computer skills.
  • Capacity to learn, synthesize information, and make critical judgments.
  • Ability to work independently and prioritize patient and family needs.

Nice-to-haves

  • Experience in a hospital setting.
  • Familiarity with AIDET patient communication standards.
