About the position

The RN Patient Navigator (NPN) helps coordinate services among medical personnel. Navigators provide support and guidance, linking patients to existing health care services. The NPN is a nurse who is familiar and experienced with oncology care and will be physically available to the patient and family. The NPN acts as clinician, care coordinator, educator and counselor. The NPN works collaboratively within an interdisciplinary health team and is responsible for providing comprehensive and age-appropriate care to both well and/or ill patients. The NPN educates patients and/or intervenes in acute and chronic illness.

Responsibilities

• Coordinate services among medical personnel and link patients to existing health care services.
• Act as clinician, care coordinator, educator, and counselor for patients and families.
• Provide comprehensive and age-appropriate care to both well and ill patients.
• Educate patients and intervene in acute and chronic illness.
• Facilitate communication between patients/families and the interdisciplinary team.
• Participate in monthly Cancer Committee/Tumor Board conferences.
• Manage patients on clinical trials and be aware of current open clinical trials available to patients.
• Provide psychological support to patients and families throughout the disease process.

Requirements

• Associate's degree from an accredited school of nursing required.
• 3-4 years experience providing direct patient care in an oncology or related clinical setting required.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure in Maryland required.
• CPR certification for healthcare providers from AHA or American Red Cross within 90 Days required.
• OCN - Oncology Nurse Certification or infusion certification (CRNI) within 180 Days required.

Nice-to-haves

• Bachelor's degree in Nursing preferred.
