About the position

The Patient Navigator role at San Ysidro Health involves providing essential support to patients by connecting them with community resources and services. This position requires both in-person and telecommunication interactions to educate and assist patients in navigating their healthcare needs, particularly focusing on vulnerable populations. The navigator will document patient interactions, identify barriers to access, and collaborate with various departments to enhance service delivery.

Responsibilities

  • Perform in-depth screening for community programs to educate and connect patients with needed services.
  • Provide services via telecommunications and in person at specific clinic sites.
  • Obtain verbal consent and/or written authorization from patients and create patient profiles on the Community Information Exchange (CIE) system.
  • Use active listening to provide high-quality confidential connections to resources and services for SYHealth patients.
  • Identify programs and resources to assist patients based on social determinants of health using the PRAPARE Tool.
  • Document all patient interactions and unmet needs into NextGen to identify service gaps.
  • Offer patients internal appointments for services such as social work and behavioral health.
  • Comply with CIE regulations and complete all necessary trainings.
  • Collaborate with the Social Services Department and participate in community events.
  • Maintain accurate records of activities and submit reports by due dates.
  • Represent SYHealth professionally and maintain positive relationships with clients and agencies.

Requirements

  • 1 to 2 years of experience working in a non-profit or equivalent education and experience.
  • Experience working with high-risk vulnerable populations and understanding socioeconomic barriers.
  • Strong computer literacy skills, proficient in Microsoft and database programs.
  • Ability to be culturally sensitive and appropriate in working with underserved communities.
  • Excellent interpersonal skills, tactful, flexible, and empathetic critical thinker.
  • Organized, able to set priorities, multitask, and exercise sound independent judgment.
