About the position

The Patient Placement Coordinator at Hackensack Meridian Health plays a crucial role in managing patient flow within the hospital. This position involves collaborating with various departments to ensure efficient patient placement decisions, maintaining communication with staff, and utilizing technology to monitor patient movement. The coordinator is responsible for ensuring patient safety and providing exceptional customer service throughout the process.

Responsibilities

• Collaborates with Capacity Manager(s), nursing supervisors/Directors, infection control staff, ED leadership, EVS & Transport leadership to ensure efficient execution of patient placement decisions.
• Maintains ongoing communication with Capacity Managers and other staff to ensure continual patient flow and that essential personnel are informed.
• Utilizes appropriate tools and technology to monitor and promote continuous patient movement.
• Ensures accurate admission, discharge, and transfer data and all patient flow measures and statistics as defined by the Capacity Management team.
• Demonstrates expertise in working with required computer applications, phone systems, and patient tracking tools.
• Interacts with all required stakeholders (nursing, physicians, EVS, transport, leaders, and team members) utilizing appropriate communication skills and tools.
• Performs daily work tasks maintaining extraordinary customer service at all times.

Requirements

• High School Graduate
• Knowledge and experience with computers, OS usage, Outlook, Word, Excel, and basic keyboarding skills
• Minimum of 1 year in a medical/acute hospital setting
• Patient Placement experience preferred
