About the position

This position is responsible for consistently monitoring patients who are at risk for potential injury or self injury taking direction and guidance from the Registered Nurse &/or Director. As appropriate, the Companion will assist with daily patient hygiene needs, ensure patient status is communicated to other members of the care team and will engage patients in activities as indicated.

Responsibilities

• Monitors patients to ensure safety from falls or self harm with attention to the maximum comfort and satisfaction of patients.
• Provides for patient safety through appropriate prevention measures (direct visualization/constant observation &/or suicide precautions &/or fall precautions.)
• Individualizes services based on physical/developmental attributes while maintaining an environment conducive to meeting safety needs.
• Engages in patient in diversion activities as appropriate.
• Encourages patient interaction and stimulation.
• Assists with patient ambulation.
• Provides assistance with food and nutrition.
• Assists RNs and PCAs with daily hygiene.
• Utilizes appropriate communication techniques.
• Observes and reports patient status changes, unsafe situations to members of the care team.
• Interacts with patient family members to share appropriate information and respond to any concerns and complaints raised.

Requirements

• High school diploma or GED equivalent.
• Previous health care experience preferred.
