About the position

The Patient Safety Monitor on the 5 North Stroke/Neuropsych/Behavioral Health unit plays a crucial role in the care delivery team, functioning under the direction of the bedside nursing team to provide delegated direct patient care interventions. This position is responsible for ensuring a safe environment for assigned patients, completing assigned tasks, monitoring patients for changes in condition, and reporting these changes to the RN or LPN. The Patient Safety Monitor is integral to the patient care team, utilizing clerical skills to maintain efficient workflow and ensure safe, quality care with a patient-centered approach that emphasizes continuity of care and patient satisfaction. Making independent clinical decisions is outside the scope of this role. The Patient Safety Monitor is expected to implement identified care plans through coordination with the interdisciplinary care team, employing strategies to promote health and wellness. This includes obtaining verbal reports from the patient's nurse regarding care needs, diagnosis, communication barriers, and special considerations. The monitor must communicate pertinent data and information in a timely manner, accurately record data collected, and notify the nurse of any condition or behavior changes. Providing patient comfort, safety, and satisfaction is paramount, which includes assisting with ambulation, positioning, and other activities of daily living (ADLs) as directed by the primary nurse. In addition, the Patient Safety Monitor promotes infection prevention through standard precautions, proper procedures in dressing changes, wound care, hand hygiene, and maintaining cleanliness in patient rooms. The role requires continuous observation of the patient’s behavior and activity, ensuring that the patient is never left alone or out of sight unless specifically instructed by the nurse. The monitor also assists with lifting and moving patients as needed, fosters a respectful environment that encourages the exchange of ideas, and integrates ethics into all aspects of practice. Commitment to lifelong learning and participation in the evolution of the professional practice model is also expected.

Responsibilities

  • Provide delegated direct patient care interventions under the direction of the nursing team.
  • Ensure a safe environment for assigned patients and complete assigned tasks.
  • Monitor patients for changes in condition and report these changes to the RN/LPN.
  • Document ongoing monitoring of patients at required intervals per orders and nursing direction.
  • Implement identified care plans through coordination with the interdisciplinary care team.
  • Obtain verbal reports from the patient's nurse regarding care needs and special considerations.
  • Communicate pertinent data and information relative to the patient in a timely manner.
  • Accurately record data collected and notify the nurse of any condition or behavior changes.
  • Assist in ambulation, positioning, and other ADLs as directed by the primary nurse.
  • Promote infection prevention through standard precautions and cleanliness of patient rooms.
  • Continuously observe the patient's behavior and activity, ensuring they are never left alone unless instructed by the nurse.
  • Assist with lifting and movement of patients as directed by the nurse.
  • Foster a mutually respectful environment that encourages the exchange of ideas and supports professional relationships.

Requirements

  • Must successfully complete Safety Monitor/Patient Sitter competency checklist within 90 days of hire.
  • Must complete non-violent crisis prevention (Mandt) training with orientation and annually.
  • 1 year of customer service experience is required.
  • Direct patient care experience is preferred.
  • Demonstrates awareness and sensitivity to the rights of patients and significant others.
  • Ability to formulate positive working relationships with all healthcare customers.
  • Demonstrates competency in selected psychomotor skills.
  • Maintains confidentiality of all patient information.
  • Basic Life Support (BLS/RQI) certification required within 90 days of hire.
