About the position

The Patient Safety Monitor will be an integral member of the care delivery team, functioning under the direction of the bedside nursing team to provide delegated direct patient care interventions. This role is crucial in ensuring a safe environment for assigned patients, monitoring their conditions, and reporting any changes to the RN or LPN. The Patient Safety Monitor is responsible for documenting ongoing patient monitoring at required intervals as per orders and nursing direction. This position emphasizes a patient-centered approach, focusing on continuity of care and patient satisfaction. The role does not involve making independent clinical decisions but rather supports the nursing staff in delivering quality care. In this position, the Patient Safety Monitor will implement care plans through coordination with the interdisciplinary care team, employing strategies to promote health and wellness. The monitor will obtain verbal reports from the patient's nurse regarding care needs, diagnoses, communication barriers, and special considerations. Timely communication of pertinent data and information related to the patient is essential, as is the accurate and legible recording of data collected. The monitor will assist in providing patient comfort, safety, and satisfaction, including helping with ambulation and activities of daily living (ADLs) as directed by the primary nurse. The Patient Safety Monitor will also promote infection prevention through standard precautions and maintain cleanliness in patient rooms. The role requires continuous observation of the patient’s behavior and activity, ensuring that the patient is never left alone unless specifically instructed by the nurse. The monitor will assist with lifting and moving patients as needed and will foster a respectful environment that encourages the exchange of ideas and supports professional relationships. Responsibilities

  • Provide delegated direct patient care interventions under the direction of the nursing team.
  • Monitor patients for changes in condition and report to the RN/LPN.
  • Document ongoing monitoring of patients at required intervals per orders and nursing direction.
  • Assist in ambulation, positioning, and other ADLs as directed by the primary nurse.
  • Promote infection prevention through standard precautions and cleanliness of patient areas.
  • Communicate pertinent data and information relative to the patient in a timely manner.
  • Maintain confidentiality of all patient information and demonstrate sensitivity to patient rights.
  • Create caring relationships with patients, taking time for uninterrupted moments and displaying respect.

Requirements

  • Must successfully complete Safety Monitor/Patient Sitter competency checklist within 90 days of hire.
  • Must complete non-violent crisis prevention (Mandt) training with orientation and annually.
  • 1 year of customer service experience is required.
  • Direct patient care experience is preferred.
  • Basic Life Support (BLS/RQI) certification required within 90 days of hire.
