About the position
The PDN RN/LVN position at VIVA Pediatrics involves providing specialized home health care for pediatric patients, particularly focusing on those with complex medical needs such as G-tubes, tracheostomies, and ventilators. The role emphasizes the importance of assessing patient needs, implementing care plans, and ensuring compliance with health regulations while maintaining effective communication with families and healthcare teams.
Responsibilities
- Assess home health patients to identify their physical, psycho-social, and environmental needs.
- Implement, develop, and document the plan of care to ensure quality and continuity of care.
- Provide care utilizing infection control measures that protect both the staff and the patient.
- Ensure nursing care reflects compliance with State, Federal & Local regulations.
- Comply with all VIVA Pediatrics policies and procedures and promote the Agency philosophy.
- Provide effective communication to patients, families, team members, and other healthcare professionals.
- Notify supervising nurse of changes in the client's condition immediately.
- Provide health education to clients and/or caregivers.
- Document interim changes of the plan of care in a timely fashion.
Requirements
- At least 1 year of work experience or 6 months pediatric home health as a licensed RN or LVN.
- CPR Certification.
- Valid Driver's License.
